Nutrition Program for the Elderly

Home Delivered Meals

The Home Delivered Meals Program (HDM) is designed to provide temporary or permanent homebound elderly persons with at least one-third (1/3) of their recommended daily nutritional requirements by delivering a hot, nutritious meal to the elderly in their own home Monday through Friday. Frozen meals are also available to clients for night, weekend, and holiday meals. In addition to the meals, our elderly clients are provided other supportive services such as information and referral, nutrition education, friendly visiting, diet counseling, and transportation. The Georgia Department of Human Resources, Division of Aging Services, through the Heart of Georgia Altamaha Area Agency on Aging and Medicaid funds the program.   The United Way of Appling County, the United Way of Toombs, Montgomery & Wheeler counties, the United Way of SouthGeorgia, various city and county governments, local churches, civic clubs, and private donations help to enhance this program. Concerted Services, Inc. also provides Home Delivered Meals in Bulloch County.  The Georgia Department of Human Resources, Division of Aging Services, through the Coastal Georgia Altamaha Area Agency on Aging and Medicaid funds the program.    The United Way of Southeast Georgia, Bulloch County Board of Commissioners, Bulloch Hospital Authority, and private donations help to enhance this program.  If you are interested in receiving Home Delivered Meals in Bulloch County, please call (912) 489-1604.

Home delivered meals may be provided only to persons age 60 or older whose functional impairments prevent them from participating in a congregate meals program, or who provide care to a dependent, disabled person in the home, to the extent that they cannot leave the person to attend a congregate site, or one of the following:

(1)  The spouse/caregivers of eligible older person, regardless of age or condition, may receive a home delivered meals if receipt of the meal is in the best interest of the homebound older person; and

(2)  Non-elderly individuals with disabilities who are a member of a household with an elderly person eligible for home delivered meals may recive a home delivered meal.

Congregate Meals

This program brings together senior citizens sixty (60) years of age or older into a multi-purpose senior center where they enjoy various activities and receive noonday meals five days a week. Frozen meals are also available to clients for night, weekend, and holiday meals. In addition to meals, the program also offers transportation, information sessions, referral services, home management, housing assistance, escort services, telephone reassurance, nutrition education, individual diet counseling, recreational activities, arts and crafts, friendly visiting, shopping assistance, and field trips.
