Elderly Services Program
A variety of programs are operated by Concerted
Services, Inc. for the benefit of senior citizens.
Programs operated by the Elderly Services Program
through contract with the Area Agency on Aging (AAA),
Medicaid, United Way, and private pay. These programs
are:
Community Care Services Program:
The Community Care Services Program (CCSP) is designed
to assist eligible individuals in continuing to live in
the community by providing cost effective community and
home based services as an alternative to a long term
care facility. Concerted Services, Inc. provides the
assessment, care management, and brokering of health
care services which match the needs of the individual.
Services that are brokered to various providers in the
community are: Alternative Living Services, Adult Day
Health, Emergency Response Services, Home Delivered
Meals, Out-Of-Home Respite Services, Home Delivered
Services (Nursing Care, Physical Therapy, Speech
Therapy, Occupational Therapy, Medical Social Services,
and Home Health Aide), and Personal Support Services.
Recipients of these services are already receiving
Medicaid or will be determined Medicaid eligible within
180 days after the first brokered service.
CCSP is funded by the Georgia Department of Human
Resources, Division of Aging Services through the Heart
of Georgia Altamaha Area Agency on Aging. The Georgia
Department of Medical Assistance funds the individual
cost of the services the client receives and pays the
various providers directly for the services they
provide. Concerted Services, Inc. provides client
assessment and care management services. The program is
offered in the following counties: Appling, Bleckley,
Candler, Dodge, Emmanuel, Evans, Jeff Davis, Johnson,
Laurens, Montgomery, Tattnall, Telfair, Toombs,
Treutlen, Wayne, Wheeler, and Wilcox.
Nutrition Program for the Elderly - Home Delivered
Meals:
The Home Delivered Meals Program (HDM) is designed to
provide temporary or permanent homebound elderly persons
with at least one-third (1/3) of their recommended daily
nutritional requirements by delivering a hot, nutritious
meal to the elderly in their own home Monday through
Friday. Frozen meals are also available to clients for
night, weekend, and holiday meals. In addition to the
meals, the elderly client is provided with other
supportive services such as information and referral,
nutrition education, friendly visiting, diet counseling,
and transportation. The Georgia Department of Human
Resources, Division of Aging Services through the Heart
of Georgia Altamaha Area Agency on Aging funds the
program. United Way, various city and county
governments, local churches, civic clubs, and private
donations help to enhance this program. These services
are provided in the following counties: Appling,
Candler, Evans, Jeff Davis, Tattnall, Toombs, and Wayne.
Nutrition Program for the Elderly – Congregate Meals:
This program brings together senior citizens sixty (60)
years of age or older into a multi-purpose senor center
where they enjoy various activities and receive noonday
meals five days a week. Frozen meals are also available
to clients for night, weekend, and holiday meals. In
addition to meals, the program also offers
transportation, information sessions, referral services,
home management, housing assistance, escort services,
telephone reassurance, nutrition education, individual
diet counseling, recreational activities, arts and
crafts, friendly visiting, shopping assistance, and
various field trips. The Georgia Department of Human

program. The United Way, various city and county
governments, local churches, civic clubs, and private
donations help to enhance this program. These services
are provided in the following counties: Appling,
Candler, Evans, Jeff Davis, Tattnall, Toombs, and Wayne.
NORC Program:
Concerted Services, Inc. began operating the Naturally
Occurring Retirement Communities (NORC) Program in
Candler County on August 1, 2007. Wherever there’s a
higher than usual density of older adults, a Naturally
Occurring Retirement Community (NORC) can take root. The
NORC model is flexible and innovative. NORCs build the
community’s capacity to support seniors as they age in
place. NORCs help seniors remain where they want to be
…. in their own communities and promote healthy aging.
The Candler County NORC program provides services to
seniors, age 60 and over, who meet specific guidelines
required by the program. NORCs can offer a flexible
array of services such as home modification and repair,
vouchers to lower the cost of services, yard work and
chore services, social programs and outing. NORCs
generate a culture of vitality and independence that can
add years to life. The program is funded through the
Jewish Federation of Greater Atlanta and the Department
of Human Resources.
